What We're Looking For: This position provides overall coordination and oversight of developing and supporting a community for all members receiving services for assigned regions in the Trillium MCO/Local Management Entity catchment area. This position ensures all program requirements are met and maintained. This position acts as the primary contact for other systems in the catchment area who work in partnership to support the identified population. In addition, this position will represent Trillium at public meetings related to any and all services and populations covered by Trillium. This position will assist with program development, consultation, resource management, coordination of other services, and agencies to best meet the needs of the persons served by various stakeholders in the community. This position involves frequent travel within the assigned region, and may work after hours and weekends on occasion when necessary to meet the needs of the organization. On a typical day, you might:

  • Coordinates and/or supports stakeholder meetings held in the community or virtually
  • Attend staff meetings of partner agencies to explain the changes and updates in the Behavioral Health and I/DD system as well as promote best practices as a Tailored Plan.
  • Support and educate stakeholders on funding sources and state and federal guidelines.
  • Work with provider agencies and Community Stakeholders to encourage the the whole person care approach to physical and behavioral health services, and Person-Centered Planning process, and provide technical assistance as needed.
  • Support the 5 Regional CFAC's with any administrative duties and give monthly updates.
  • Work collaboratively with the other departments
