Full time Hospice Admission Liaison RN
The Community Hospice has a rewarding opportunity for a full-time RN to complete Hospice Admissions primarily in our nursing home settings throughout the 8 counties we serve at The Community Hospice-from Saratoga to Catskill.
HOURS : 8am-4:30pm (can be flexible). Position does require one Saturday per month.

What you will do:
The RN coordinates with the patient's care team in the LTC setting their medical director and our hospice medical directors to confirm eligibility for hospice services.
Will go to local nursing homes to complete initial assessment of patient/family needs and begin implementation including coordination and support from our interdisciplinary team which includes social workers, chaplains, volunteers, complementary therapists, certified nursing assistants, and more!
Ensures a smooth hand-off to the Hospice Case Manager.
Corporate office is 445 New Karner Road-Albany NY with satellite offices in Catskill and Saratoga NY. Admissions are across 8 counties: Columbia, Greene, Albany, Rensselaer, Schenectady, Saratoga, Washington. Your region is considered in patient assignment.
We serve 42 different Nursing homes within our capture areas
What you will need:
NYS RN Licensure
ASN required, BSN preferred
At least 1 year experience as an RN working in LTC setting highly preferred
Homecare experience required
Hospice experience preferred
Reliable vehicle and driver license
Excellent communication skills. Must be able to communicate effectively with patients, physicians, and peers.
Must be able to document key information concisely and accurately
Strong computer skills
Strong organizational and time management skills
Pay Range: $33.10-$45.75
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
