Description

  • Responsible for the development and implementation of processes that support the organizations patient safety priorities, coordination/oversight of the completion of root cause & proactive analyses and tracking of appropriate organizational follow up and monitoring.

  • This position also has oversight of the electronic incident reporting system.

  • System coordinator maintains current knowledge of CMS, DCH and TJC requirements, provides timely communication to system leaders and operating unit leader and serves a subject matter expert to all operating units.

  • Standardize patient safety plan & processes across all EHC operating units. Identifies and implements best practices for performing FMEAs, RCAs and other analyses of patient safety events.

  • Reports trends and patterns based on events in SAFE. Sharing lessons learned with all operating units.

  • In collaboration with the EHC PSO, participates in the development of the Patient Safety Plan and coordinates efforts to move concepts to operations.

  • Participates in the annual evaluation of the Patient Safety Plan, annual goal setting and report to the Board.

  • Develops and implements on-boarding training/education plans and modules for new EHC Patient Safety staff and Leadership.

  • Develops and implements facilitation skills curriculum and training on FMEAs, RCAs and Human Factors.

  • Provides professional development for OU Patient Safety Specialist/Coordinators.

  • Assist Quality & Patient Safety Directors in hiring OU Patient Safety Specialists.

  • Provides OU consultant services to assure compliance with patient safety standards and processes.

  • Coordinates and oversees the completion of investigation, analysis and follow up of adverse clinical events, errors, outcomes, hazards and/or clinical quality or service topics in collaboration with the OU Patient Safety Specialist.

  • Provides reports and system summaries to system leaders and operation unit leaders.

  • Coordinates/facilitates information flow to/from Committees (CSCCo), Chief Quality Officers and Chief Medical Officers. Functions as liaison with Risk Management, Medication Safety designee, key process owners, and Nursing leadership to coordinate information flow.

  • Coaches/influences individual leaders to achieve measurable improvements in patient safety and incorporate patient safety into daily work.

  • Coordinates formal sentinel event review, reporting and follow up process as needed for external agencies (CMS, DCH, TJC, MCO).

  • Serves as point person for submitting CMS/DCH reportable events.

  • Manages system-level patient safety employees.

  • Manager of the electronic incident reporting database

Minimum Qualifications:

  • Master's degree from an accredited college or university in nursing, other health-related field or health administration.

  • Knowledge of healthcare QM/PI and/or Risk Management concepts, theory & application

  • Previous hospital clinical experience required.

  • Three years experience in QM/PI and/or Risk Management programs within a hospital setting, root cause analysis methodology, team/committee facilitation and TJC/CMS / DHR accreditation

  • Sentinel event reporting experience preferred.

  • Must be a licensed RN.

  • Must obtain Georgia Licensure within 6 months of hire.

  • In lieu of a Master's degree, candidates with 10 years leadership experience in QM/PI and/or Risk Management may be considered.

Preferred Qualifications:

  • Documented experience in improving patient safety and building a safety culture.

-Experience in establishing close working relationships with physicians and nursing leaders.
