Position: License Practitioner Nurse

The LPN performs various aspects of the client medical care under the supervision of a Registered Nurse . The LPN directly provides and/or coordinates health services and health-related activities for clients participating in or completing treatment.

Position Status: This is a part-time position. 19 hrs

Job Location: Women's Recovery Residence, Brentwood, NY 11717

Job Scope:
  • Administer/or monitor all medication calls and health calls as scheduled under the supervision of a registered nurse.

  • Work with both clients and guardians to schedule medical appointments to assist clients in maintaining optimal health and conduct any follow up as required. (Referrals documentation medical consent, etc.)

  • Work with Agency psychiatrist regarding client medication and pharmacy refills to ensure prescriptions are filled/re-filled in a timely manner; contacting guardians regarding parent consent for new medications and medication changes and assisting with Order Connect data entry and monitoring.

  • Provide on-site health services to address emerging and/or current health needs of client(s) under the supervision of a registered nurse.

  • In conjunction with facility managers and medical team the LPN will provide feedback to health-related policies and procedures to ensure a safe and healthy environment.

  • In a health emergency, provide immediate first aid to individual(s) in need and monitor situation until emergency services arrives, assessing them of the situation. When necessary, escort client(s) to emergency and/or health facility.

  • Performs Clinical procedures such as dressing changes and immunizations (with additional training)

  • Obtained accurate medical data including but not limited to a nursing intake/health history, tobacco use history and cessation plan and communicable disease risk of substance abusers these data will help the agency Psychiatric and Medical Doctors to determinate the individualized medical treatment.

  • Prepare new clients for their admission physical examination, and when requested by the medical director the LPN may be present during physical exams.

  • Reports and records results of nursing care and make all significant information known immediately to the Primary Registered Nurse / Medical Director. Keep staff informed both verbally and in writing of significant matter related to the clients.

  • Maintains adequate inventory of all supplies and prepare exam room at the beginning and end of the day and in between each patient contact.

  • Perform variety of office responsibilities such as: Manage inventory of pharmaceutical, breakdown of inactive medical charts, File of all paperwork on a daily basis.

  • Ensures all medications and treatments are completed and documented prior to the end of the shift. Provides shift report to the following shift.

  • Communicating (in writing as well as orally) clearly and concisely

  • Maintaining appropriate boundaries, ethics and professionalism; complying with agency, OASAS and other professional code(s) of ethics; seeking supervision when challenges arise.

  • NYS Licensed Practical Nurse (LPN)

  • Previous experience working as a Licensed Practical Nurse (LPN) in a residential substance use disorder treatment setting preferred.

  • Previous experience working as a Licensed Practical Nurse (LPN) working in a related.

  • Treatment, therapeutic community, community residence, and/or mental health setting working directly with special populations required.

    Work Environment

    This job operates in a professional office environment and outside in the field. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Must be able to travel among assigned sites and locations as needed.
