BronxCare Health System Lead Family Support Worker, Department of Obstetrics & Gynecology in Bronx, New York

Overview

The Lead Family Support Worker of the Healthy Families Program will report directly to the Program Supervisor. In addition, will maintain a caseload, will also play an important leadership role in the program by orienting and training new staff, coordinating staff and participant training sessions and family support/educational groups. Will also assist in identifying and establishing relationships with potential referral sources in the community.

Responsibilities

  • Establish and maintain positive relationships with patients, visitors, and other employees. Interacts professionally, courteously, and appropriately with patients, visitors and other employees. Behaves in a manner consistent with maintaining and furthering a positive public perception of BronxCare Health System and its employees.

  • Contributes to and participates in the Performance/Quality Improvement activities of the assigned department. Contribution and participation includes data collection, analysis, implementation of and compliance with risk management and claims activities, support of and participation in Continuous Quality Improvement (CQI) teams, consistent adherence to the specific rules and regulations of the BronxCare Health System (a) Safety and Security Policies, (b) Risk Management: Incident and Occurrence Reporting, (c) Infection Control Policies and Procedures and (d) Patient and Customer Service.

  • Responsible for initiating and maintaining regular contact with families primarily in their homes.

  • Provides intensive services and ensures completion of goals. These intensive services will focus on promoting positive parenting skills that will center on positive parent-child interaction and enhance the knowledge of child development of program participants utilizing various program activities.

  • Conducts assessment with the input of the family to determine service needs and develop goals utilizing the Family Support Plan.

  • Escorts families to agencies and medical visits when necessary.

  • Responsible for documenting all contact with families as they pertain to all service delivery in case records and in other required data collection instrument.

  • Responsible for planning and organizing events for program participants, such as employment readiness, parenting groups and the Community Pediatrics program.

  • Will train new hires on Family Support/Family Assessment Worker procedures, including: taking new workers on home visits to observe, accompanying the new worker on his/her first home visit, providing feedback and assistance in completing all program forms (Home Visit Log, IFSP, PSI, etc.), providing the new worker with information on community resources, conducting staff training and holding team meetings as needed.

  • Assist in outreach efforts by establishing relationships with community partners and potential referral sources. This includes: making presentations, attending outreach fairs, speaking with pregnant women, new families and area providers about the program, etc.

  • Will assist with special projects and perform other duties as delegated by the Program Director and Supervisor.

Qualifications

  • Experience working within childcare facility such as a Day Care Center/Head Start Program or working with high-risk families preferred.

  • Applicants who are bilingual in French, Spanish and/or African languages are preferred.
