Solvista Health Care Navigator in Canon City, Colorado

General Purpose The Care Navigator provides a partnership approach to healthcare and community-based services, to support medical, social, developmental, behavioral, educational, and financial needs to optimize health and wellness.

Essential Duties/Responsibilities Level I Works closely as part of a clinical team following the treatment plan and documenting appropriately. Works closely within the client support services team to ensure the highest level of client care. Works with team members and supervisor to ensure management of cases is appropriate. Performs case/care coordination tasks as assigned in a timely manner. Completes clinical documentation and navigation of Electronic Health Record. Maintains professional demeanor and confidentiality when interacting with co-workers and clients. Serves as a source of general information regarding care/case management, Solvista, and community partners. Participates in required learning activities and attends staff meetings. Completes all trainings, including, but not limited to, Bridges out of Poverty, Mental Health First Aid, and Motivational Interviewing. Maintains regular and predictable attendance of assigned work shifts. Effectively communicates with other team members. Fully understands client's rights and client responsibilities. Performs other duties as assigned.

Level II Maintains all Level I requirements. Demonstrates operational knowledge of resources and support of clients. Demonstrates critical thinking skills and autonomy as it relates to client care. Exhibits effective time management skills and ability to multitask. Communicates case management perspective with diverse clinical team members. Provides constructive consultation and support to other team members regarding ethical decision-making, healthy professional boundaries, and the level of client support needed. Successfully completes home or community visits with clients. Reviews documentation of team members and mentors as needed to build skill level. Participates in treatment plan development as required. Demonstrates full understanding of care coordination spreadsheets, deliverables, and documentation. Coordinates care with primary care providers and specialty clinics to meet member needs specific to their disease condition and health care utilization. Facilitates bachelor's level support groups in conjunction with clinical treatment team and supervisor. Demonstrates competency in providing regular skills-based interventions appropriate to case/care coordinator role supporting clinical treatment plan. Demonstrates a full understanding of multi system approach to care. Actively pursues additional education and certification in case/care coordination field. Maintains positive community partner relationships. Consults with supervisor when appropriate.

Level III Completes and maintains all level I and level II requirements. Analyze complex, and sometimes contradictory information to effectively solve problems. Make appropriate and timely decisions that keep clients and the team moving forward. Gains the confidence of co-workers through honesty, integrity, and authenticity. Actively learns through experimentation when tackling new problems using both successes and failures. Assists in the coordination of department goals and work assignments of other case/care coordinators. Provides data-based input for budget, staffing and assignments in support of department leaders. Represents agency and is primary contact for at least one community partner meeting identifying resources and supports for client service needs. Acts as primary liaison to a community group. Designs and participates in specialty program development and maintenance. Utilizes experience and education to design client intervention strategies. Acts as a mentor to Level I care navigators. Performs other duties as assigned.
