Guthrie Social Worker - Social Work - Full Time in Corning, New York

Position Summary

Responsible for assessing and providing psycho-social needs of patients to enhance their optimal level of health and/or recovery. Provide interventions for various populations and serve as a resource for the patient and health care team to assist in establishing treatment goals and provide support/intervention in crisis situations.

Experience

Minimum of two years post graduate experience as social worker preferred.

Education

Graduate of a master’s degree program in Social Work.

Licenses

  • License in Social Work required.

Essential Functions

  • Provides psycho-social support to patients and families

  • Coordinates care, transportation, and other services with patient and family.

  • Continuously assesses psycho-social needs of the patient and family throughout care.

  • Assesses the social and emotional needs related to the patient’s illness, need for care, response to treatment, and adjustment to care being provided

  • Develops meaningful relationship with patient and families that fosters dialogue and trust.

  • Assesses patients’ medical and nursing requirements, home situation, financial resources, and community needs.

  • Helps provide community and Guthrie resources available to patients in need.

  • Collaborates with other health care disciplines to ensure quality care is provided.

  • Research available funding for patients who qualify for assistance.
