Description

Trio Hospice / Jet Health, Inc,is seeking a compassionate and dedicatedPRN / Per DiemHospice Chaplainto join our hospice team serving our patients in theDenton, TXand surrounding areas.

The Hospice Chaplain provides spiritual support and counseling to patients, families, and caregivers, helping them cope with terminal illness and end-of-life issues. This role is an integral part of the interdisciplinary hospice care team, ensuring that patients' spiritual needs are met with sensitivity and respect.

Requirements

  • Education: Master of Divinity, preferred.

  • Licensure: Per state guidelines, if any

  • Experience: One (1) year experience as a Chaplain in a health care institution preferred.

  • Skills: Working knowledge of community spiritual and other resources. Good interpersonal skills. Transportation: Reliable transportation. Current auto liability insurance and drivers license if personal vehicle is used for work.

Environmental and Working Conditions:

  • Works in patient homes and other healthcare facilities under various conditions. Occasional exposure to infectious diseases; ability to travel locally; may have some exposure to unpleasant weather.

Physical and Mental Effort:

  • Prolonged standing, walking and sitting required; requires working under occasionally stressful conditions to meet deadlines and patient/family needs, must be able to make quick decisions, function as a facilitator to meet patient/family spiritual needs.

Essential Functions:

  • Provide spiritual care and support to hospice patients and their families.

  • Conduct spiritual assessments and develop individualized care plans.

  • Offer counseling and emotional support to patients and families facing end-of-life issues.

  • Collaborate with the interdisciplinary team to ensure holistic patient care.

  • Facilitate religious services, rituals, and practices as requested by patients and families.

  • Provide grief support and bereavement counseling to families and caregivers.

  • Maintain accurate documentation of spiritual care services provided.

  • Participate in interdisciplinary team meetings and contribute to care planning.

  • Educate staff and volunteers on spiritual care principles and practices.

  • Ensure services are provided in accordance with patient and family preferences and beliefs.
