The Colorado Mental Health Hospital in Fort Logan (CMHHiFL) is a138 bed psychiatric facility located in Denver that is dedicated to meeting the needs of the community by providing mental health treatment to a civil and forensic adult population in the metro area. The mission of CMHHIFL is to provide the highest quality mental health services to adults with complex, serious, and persistent mental illness within the resources available

About this Work Unit:

The Quality Management Department exists to provide a structure to support the development and implementation of effective systems to promote and facilitate quality care in a safe environment and to ensure accreditation by the Joint Commission and certification by the Centers for Medicare/Medicaid Services are maintained. The Quality Management Department guides the Colorado Mental Health Hospital in Fort Logan’s efforts to meet or exceed customer expectations by maintaining excellence in patient care and to identify opportunities for improvement through analysis, interpretation and dissemination of data-based information that enables hospital leadership to utilize resources in an efficient manner and to operate effectively under legislative mandates and administrative policies.

Description of Job

This position will be responsible for the clinical risk management activities at CMHHIFL. This position will be tasked to ensure a proactive approach to Incident and Risk management is accomplished across the Hospital with consistent application of the Hospitals Risk and Incident Management Policies and Guidance. This will involve implementation of the Hospital Risk and Incident management processes including: risk assessment, incident management, and the development of support systems and processes for incident management including communication and escalation of serious incidents (internally and to external agencies e.g., TJC, CMS, CDPHE).

Primary Job Duties:

Regulatory Compliance and Readiness:

Serve as a resource hospital-wide for standards, regulations, compliance trends, and propose initiatives by providing training, consultation, conceptual framework and structure to hospital staff. Work with hospital management and staff to determine the level of compliance with regulatory standards and rules and develop plans for compliance. Work with the Joint Commission and other accrediting and regulatory bodies to report and correct problems. Monitor various publications, websites, and other media for current standard changes and issues. In collaboration with the Quality Department this position develops the process for the regulatory survey visits, follow up, and corrective action plan submission. Monitor the Hospital’s Plans of Action and collaborate with Directors/Managers to ensure these plans meet regulatory requirements

Clinical Risk Management:

Responsible for the clinical risk management activities at CMHHIFL including the analysis of clinical and administrative systems, evaluation of processes, and establishing and maintaining standards to detect, monitor, assess, mitigate, and prevent risks. Responsible for determining what recommendations to make to reduce, prevent or eliminate risks identified. This position will develop and implement risk management policies and procedures, complete targeted risk assessments based on internal, local, and national hospital trends. This position collaborates with department leaders in order to improve the management and treatment of patients; improve management of employee-related injuries; provide a database for incident analysis, correction, and continuing education.

Workplace Violence Administrator:

Serves as the chair of the workplace violence committee, cultivates safer work environments by developing clear workplace violence protocols, captures and tracks workplace violence incidents from all available sources, including incident reports, IOJ’s, safety reports, human resources, and employee surveys. This position uses this data to inform quality improvement initiatives to reduce incidences of workplace violence. These initiatives may involve changes to the physical work environment, such as enhanced security and better exit routes, as well as changes to work practices or administrative procedures, such as developing workplace violence response teams

Minimum Qualifications:

Experience Only:

Six (6) years of relevant experience in an occupation related to the work assigned to

this position

OR

Education and Experience:

A combination of related education and/or relevant experience in an occupation

related to the work assigned to this position equal to six (6) years

Preferred Qualifications:

  • Experience and knowledge of Joint Commission and/or CDPHE standards

  • Experience with CMS regulations

  • Hospital (Psychiatric and/or Medical) experience

  • Experience in working with and analyzing incidents

Conditions of Employment:

The successful candidate in this position must be willing and available to submit to the following conditions:

  • Due to the juvenile population served at this facility, incumbents of this position must be at least 21 years of age.

  • Former State employees who were disciplinarily terminated or resigned in lieu of termination must (1) disclose that information on the application (2) provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position, (3) provide the employee number from the applicant’s prior State employment. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination and failure to provide this information will disqualify the applicant from future State employment with CDHS.
