Description of Job

What you'll be doing:

The Case Management Eligibility Coordinator position is the Department Staff Authority for managing the Department’s response to stakeholders’ inquiries regarding the LTSS eligibility process, application, procedures and operations. Position is responsible for working with Department contractors, providers, and state agencies to ensure Coloradans with a need for long-term services and supports are able to access resources easily. This position is the staff authority who manages the functions of the LTSS eligibility process over contracts and members. This position operates within a strategic framework, set by upper management and supervisors, for how eligibility questions are interpreted and responded. They are responsible for determining the process of answering those requests and any updates to regulation or operational policies that are required to ensure accessibility of services.

This position provides training and technical support to outside organizations and increases the accessibility of waivers and state funded services provided through the Department. This position empowers Case Management Agencies, hospitals, schools and other entities with the tools to assist individuals making transitions from one program or environment to another in a person-centered and efficient manner, eliminating barriers and delays. Leads stabilization and enhancement coordination activities of the CCM system program content in coordination with the Division and the Health Information Office (HIO) representing OCLs interests on behalf of the program. Develops, monitors and modifies the contractor scope of work and assures state procurement rules are complied with.

MINIMUM QUALIFICATIONS:

There are two ways to qualify for this position:

Option 1:

Experience Only:

Seven (7) years of related professional experience in Long-term Services and Supports (LTSS) systems performing case management or care coordination, public administration, healthcare administration, policy development and implementation, regulation and compliance, project management, or continuous policy improvement.

Option 2:

Education and Experience:

A bachelors degreefrom an accredited institution in Psychology, Healthcare Administration, Government Policy, Social Work, Sociology, Political Science or a field of study closely related to the work assignment,

AND

Three (3) years of professional experiencein Long-term Services and Supports (LTSS) systems performing case management or care coordination, public administration, healthcare administration, policy development and implementation, regulation and compliance, project management, or continuous policy improvement.

Appropriate education will substitute for the required experience on a year-for-year basis. Relevant education will be calculated as the following:

  • An associate will count as two years of experience

  • A bachelors will count as four years of experience

  • A masters degree will count for six years of experience

  • A doctorate or juris doctorate degree will count for seven years of experience

PREFERRED QUALIFICATIONS:

  • Prior state experience related to the duties of this position

  • Experience researching, interpreting and applying complex statutes and regulations for Long-term services and supports as well as Home and Community Based Services waivers.

  • Expert level experience with long-term care services and supports and Home and Community Based Services eligibility

  • Expert level experience with grievances processes and managing Health First Colorado stakeholders

  • Master’s Degree in closely related field

DEFINITION OF PROFESSIONAL EXPERIENCE: Work that involves exercising discretion, analytical skill, judgment, personal accountability, and responsibility for creating, developing, integrating, applying, and sharing an organized body of knowledge that characteristically is uniquely acquired through an intense education or training regimen at a recognized college or university; equivalent to the curriculum requirements for a bachelor's or higher degree with major study in or pertinent to the specialized field; and continuously studied to explore, extend, and use additional discoveries, interpretations, and application and to improve data, materials, equipment, applications and methods.

Highly Desired Knowledge, Skills and Abilities :

  • Working knowledge of Medicaid Home and Community Based Services and associated regulations and best practices

  • Working knowledge of other state agencies and their programs, such as the Department of Human Services, Department of Corrections, Department of Education and the Department of Public Health and Environment.

  • Working knowledge of Medicaid state plan benefits and Medicaid operations.

  • Advanced Technical Writing, including but not limited to standard operating procedures, contract language, scope of work, and memorandums.

  • Experience facilitating complaint resolution processes between contractors, Medicaid members, families, advocates and other stakeholders. Ability to investigate root causes of eligibility issues, outline potential solutions and work towards resolution all while communicating updates to the team.

  • Ability to be a liaison between divisions, teams and state departments..

  • Advanced communication skills, i.e. conflict resolution, negotiation, solution-focused facilitation, and problem solving.

  • Interpersonal skills, including to but not limited to, clear and concise communication; attentive to multiple details; careful listening; the ability to collaborate with diverse groups and ability to adapt to their needs; independently motivated and self-starting.

  • Creative thinking approach to problem solving.

CONDITIONS OF EMPLOYMENT:

  • All positions at HCPF are security sensitive positions and require that the individuals undergo a criminal record background check as a condition of employment.
