Optum is seeking a Nurse Practitioner to join our HouseCalls team in Wayne County, MI. Optum is a clinician-led care organization, that is creating a seamless health journey for patients across the care continuum.

As a member of the broader Home and Community Care team, you’ll provide annual clinical assessments to patients in the comfort of their homes. This important preventive care helps identify and reduce health risks for patients, in addition to coordinating appropriate follow-up care to improve health and well-being.

Primary Responsibilities:

  • Coaches and evaluates APCs with the HouseCalls visit

  • Partners with CTMs to ensure clinical practice adhere to HouseCalls’ clinical model

  • Utilizes advanced clinical nursing expertise, knowledge of geriatric/chronic disease management to provide coaching, mentoring and role-modeling to new and existing NPs

  • Identifies and supports developmental clinical needs of NPs to achieve successful outcomes

  • Ensures appropriate implementation and adoption of clinical and quality initiatives as related to home visits

  • Fosters and develops a culture of clinical expertise

  • Provides visit feedback to NPs

  • Provides a summary of the visit to the CTM

  • Works with DCO and CTM to determine percent of time in the field completing member visits

Required Qualifications:

  • Undergraduate degree or equivalent experience

  • Required HouseCalls NP Travel visits throughout the State of Michigan approximately 50% annually

  • Successful completion of an accredited Nurse Practitioner program: Masters or Doctoral prepared

  • GNP, FNP, ANP, or ACNP board certified or eligible with an unencumbered license to practice in assigned state

  • 5+ years of practice experience in a clinical setting

Preferred Qualifications:

  • Experience in a home setting

  • Experience in working within a managed care environment

  • Knowledge of HEDIS and STAR quality measures

  • Proficiency in Microsoft Office products and applications

  • Excellent organizational and administrative skills

  • Solid written and verbal communication: Clearly expresses ideas, but is tactfully sensitive to opposing viewpoints

The salary range for this role is $89,800 to $176,700 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
