Helping Hands Home Care Service Occupational Therapist Assistant - PRN in Hermitage, Pennsylvania

Helping Hands is expanding and looking for Occupational Therapist Assistant to be part of our company growth and diversification.  Helping Hands is looking for part-time or PRN Occupational Therapist Assistant. Overview The Certified Occupational Therapy Assistant is responsible for providing rehabilitative care occupational therapy treatments/services for home health patients needing assistance and training in ambulation, transferring, gait training and home exercise programs. The Certified Occupational Therapy Assistant works under the supervision of a qualified Occupational Therapist in accordance with the Home Health Plan of Care. The Certified Occupational Therapy Assistant works as part of the interdisciplinary home care team including RN, PT, PTA, SLP, MSW and Home Health Aides. PRIMARY RESPONSIBILITIES * Provides occupational therapy services to patients according to the home health plan of care and under the supervision of a qualified Occupational Therapist. This may include, but is not limited to: * Performs occupational therapy procedures in the patient’s home. * Assists the Occupational Therapist in the evaluation process through observation and/or clinically derived surveys in areas including development of perceptual motor skills, motor coordination and sensory motor development and processing. * Assists Occupational Therapist in developing a treatment program aimed at improving the identified areas of function. * Instructs, motivates and assists patients in learning and improving functional activities. * Teaches basic principles of body mechanics to patients and families. * Instructs patients and family members on the proper use of prosthetic devices and in the use and care of therapeutic appliances. * Observes patient’s progress and response to treatment and reports findings to the Occupational Therapist. * Reports changes in the patient’s condition to the supervising Occupational Therapist. * Assists in preparing clinical notes and progress reports. * Participates in educating the patient/family and in inservice programs. * Modifies Plan of Treatment, as authorized by the physician, after subsequent authorization, instruction and supervision by the Occupational Therapist. * Participates in patient case conferences and team meetings. * Participates in in Helping Hands Home Care Service Agency’s Quality Assessment and Performance Improvement (QAPI) Program as requested. * Confers with the attending physician, home care team members and patient and/or family/caregivers to promote coordinated and comprehensive care. Communicate plan of care to attending physician, home care team members and patient/family and/or caregivers. * Participates in meetings and other related activities as needed and directed. * Prepares and submits clinical and progress notes in a timely manner and in accordance with Helping Hands Home Care Service Agency policies, state and federal guidelines and regulations. * Instructs patient and/or family/caregiver and other team members in the patient’s treatment regime as indicated. * Meets or exceeds delivery of Helping Hands Home Care Service Agency service standards in a consistent fashion. * Adheres to Helping Hands Home Care Service Agency’s Code of Conduct. * Complies with local, state and federal laws, Medicare regulations and established Helping Hands Home Care Service Agency company policies and procedures. PHYSICAL REQUIREMENTS * This position requires the Certified Occupational Therapy Assistant to work in a variety of environments (housing styles, cleaning and maintenance, heating and cooling) during visits to patients in their place of residence.  * Visual/hearing ability must be sufficient to comprehend written/verbal communication. * The Certified Occupational Therapy Assistant must have ability sufficient to ambulate throughout a patient's home, navigating stairs and other physical barriers typical to private residences. * The Certified Occupational Therapy Assistant must be able to operate a motor vehicle and travel to and visit in patient’s homes in varying weather and road conditions. * Must be able to deal effectively with stress. EDUCATION AND EXPERIENCE Education * Must be a graduate of an Occupational Therapy Assistant educational program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA) or its successor organizations.   Experience * Minimum of two years’ experience as a Certified Occupational Therapy Assistant with community/home health experience preferred. Licensure, Registration, and/or Certification * Must be certified by the American Occupational Therapy Association as an Occupational Therapy Assistant. * Must possess current license to practice as a Certified Occupational Therapist Assistant from the Commonwealth of Pennsylvania or made application for same. * Is eligible to take or successfully completed the entry-level certification examination for Occupational Therapy Assistants developed and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT) * Valid driver’s license and reliable transportation that is insured in accordance with Pennsylvania requirements. * Satisfactory references from employers and/or professional peers. * Satisfactory criminal background check. * Maintains current CPR certification.
