POSITION SUMMARY

Under the direction of the Social Services Director, the Social Worker will be responsible for resident assessments, creating and implementing a treatment plan in consultation with other departments, coordinating outside agencies and resources when necessary and evaluating and monitoring the resident for changes. The social worker will serve as the resident's advocate and will speak for the resident to receive the highest quality of care and services even when he/she is unable to. This position will act as a liaison between the facility and the families, providing effective communication.

 

ESSENTIAL JOB FUNCTIONS

  • Assess the resident and gather information from both the resident and family to develop a comprehensive care plan.
  • Complete all scheduled resident assessments timely and accurately in accordance with the RAI manual. Identifies cognitive, communicative, behavioral, and psychosocial needs or concerns through the assessment process and implements individualized care plans to address the identified strengths and weaknesses with measurable goals and appropriate interventions.
  • Maintain informative and accurate progress notes, adhering to professional standards regarding completion of MDS assessments, discharge planning, referrals to outside entities, psychiatric/psychology visits, service(s) provided and the resident's response to such service(s) and any resident/family concerns requiring intervention by Social Service staff.
  • Provide appropriate referrals to families and residents for outside services, including Medicare/Medicaid and other financial support services or social support services available within the community.
  • Participate in both the admission and discharge process following all proper procedures.
  • Communicate in an effective and efficient manner both within and outside the department for the highest level of quality care for the residents.
  • In the absence of the Director, assume authority over the department.
  • Work in a proactive, efficient manner, always seeking to standardize work methods.
  • Assist in developing a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified. Ensuring proper communication with all.
  • Review nurses' notes to determine if the care plan is being followed. Report problem areas to the RNAC.
  • Review and revise care plans and assessments as necessary, but at least quarterly.
  • Review complaints and grievances made by the resident and make a written/oral report to the Director indicating what action(s) were taken to resolve the complaint or grievance. Follow facility's established procedures.
  • Maintain a written record of the resident's complaints and/or grievances that indicates the action taken to resolve the complaint and the status of the complaint.
  • Provide residents with information concerning resident rights, living wills, etc. Explain as necessary.

General

  • Respond to fire alarms as required (refer to Fire Safety Manual for employees' responsibilities and those that are specific to your position.
  • Treat all residents fairly, with kindness, dignity and respect. Maintain confidentiality of all resident and facility information.
  • Report all complaints, concerns made by the resident, allegations of abuse or neglect to direct supervisor or the RN Supervisor.
  • Follow all safety procedures and precautions in the performance of your duties and when operating equipment. Report all safety violations, unsafe or hazardous conditions and any equipment issues immediately.
  • Review Corporate Compliance educational materials, comply with Corporate Compliance directives, and report any Corporate Compliance issues according to established procedures.
  • Recognize your role as part of the QAPI efforts of the organization.
  • Attend traini gs to build understand and capacity to undertake QAPI work.
  • Carry out QAPI roles and responsibilities as assigned.
  • Follow established policies and procedures in support of QAPI efforts.

     

POSITION QUALIFICATIONS

  • Ability to provide care/services in a manner supportive of the Mission and Philosophy of the Carmelite Sisters for the Aged and Infirm.
  • Speak, read, write, and understand English effectively at a level appropriate for the safe and effective performance of the job.

     

SKILLS & ABILITIES

Education:                                Minimum, bachelor's degree in social work or related field including but not limited to sociology, special education, rehabilitation counseling and psychology from accredited school

Experience:     Minimum, one (1) year supervised social work experience in a health care setting working directly with individuals
