GENERAL SUMMARY

Provides administrative support to program case management.

ESSENTIAL JOB FUNCTIONS*

  • Provides home visit relief to Nurses and Case Managers when needed.

  • Holds a small case load, and performances Case Manager role for that case load.

    Audits charts for completion.

  • Completes and updates identification sheets and photos.

  • Follows up via telephone to request missing caregiver logs/financial transaction records (FTR) and other essential case file information.

  • Tracks six month summaries and physician’s orders.

  • Tracks fire drills, home inspections, furnace inspections and safety plans.

  • Tracks caregiver trainings.

  • Files paperwork.

  • Tracks Individual Service Plans (ISP) and participant census via caregiver logs.

  • Tracks selected individuals’ financial information (RFMS).

  • Serves as key contact for Adult Foster Care determination information.

  • Completes paperwork for new program participants/caregivers.

  • Ensures adequate supply of commonly used forms.

  • Manages incoming and outgoing mail.

  • Maintains daily staff schedule. Updates the on-call list.

  • Orders supplies.

  • Maintains updated relief list.

  • Calculates caregiver respite balances and submits check requests to management for approval.

  • Provides information and referrals.

  • Serves as mandated reporter for suspected abuse and/or neglect of individuals to the Disabled Persons Protection Commission (DPPC).

PREPARATION, KNOWLEDGE, SKILLS & ABILITIES

  • High school diploma.

  • Valid driver’s license in the state of Massachusetts.

  • PC proficient.

  • Ability to work under pressure.

  • Ability to multi-task.

SUPERVISORY RESPONSIBILITY

  • No supervisory responsibility.

WORKING CONDITIONS/PHYSICAL DEMANDS

  • Ability to lift a minimum of 20 pounds in office supplies.
