THE RUNDOWN:
This position is part of the clinical team. This position will work with population health and pharmacy staff to identify and address patient barriers to medication adherence and refill coordination under the supervision of the Clinical Pharmacy Services Manager. This position will assist with other initiatives relating to pharmacy and medication as needed.
Requirements

- Provides positive impression, through words and tone, to patients, visitors, and employees presenting to the clinic areas and on the phone.
- Identify and outreach to patients with medication adherence issues to discuss possible barriers and solutions to improve adherence. Solutions include but are not limited to; medication synchronization, automatic refills, text reminders, and facilitating pouch packing as needed.
- Identify patients that would benefits from a mediation review and refer them to a clinical pharmacist.
- Update patient medication lists in the electronic medical record to include current prescriptions from outside providers that are being filled at an in-house pharmacy.
- Performs all duties, services and documentation in full compliance with CHC/SEK policies and procedures.
