Ebenezer-Lake City Care Center TCU LTC offers a robust benefits package and wages ranging from $21.92 - 23.73 / hour.
Shift differentials of $1.00 evenings and $1.50 nights
We are seeking a Nursing Assistant/Registered (CNA/NAR) to support our adult residents in our Skilled Nursing Facility in Lake City, MN! This role provides hands on care and support services for our Care Center long term care unit residents and young adult-geriatric Transitional care unit patients.
This patient support schedule includes;
75 hours in two weeks
Day shift
Every other weekend (sat/sunday) and holiday rotations

Lake City Care Center is owned by the Mayo Clinic Health System with the daily operations managed by Ebenezer. At any given time, as many as 90 people call our beautiful, vibrant and welcoming Long Term and Transitional Care Center their home. Our care center is connected to a Mayo clinic and hospital, so residents don't have to leave the building for a clinic appointment or emergency care if needed.
Assists resident based on individual care plan with personal cares such as; bathing, oral hygiene, grooming, toileting, dressing, showers and responds to call lights
Nursing Assistant may per community patient care model check and record vitals
AM/PM cares; including peri care and catheter care
May turn, lift or transfer residents with or without other persons/mechanical assistance
Observes residents and reports status to appropriate staff in a clear and concise manner
Chart or document in electronic medical record system
Seeks to resolve resident and family concerns
Receives and gives report to appropriate staff during various times in work shift
Other duties such as; order and cleanliness in rooms and work areas, reports malfunctioning equipment to maintenance department, and answers phones in a timely manner
Required:
Active on MN Nursing Assistant registry by start date
If hired into the Ebenezer Nursing Assistant Training program, granted a 4-month grace period from hire date to be active on the MN Nursing Assistant registry
Preferred:
Ability to work with a diverse and geriatric population
Communicate efficiently, with empathy and understanding for residents and coworkers
Experience working as a HHA, PCA, DSP, NA, Resident Assistant
Work experience in a customer service setting
