Job Overview:
The Sickle Cell Navigator will assist with care coordination, engage in outreach activities and serve as a liaison between patients and their families, health care providers and Community Based Organizations. The Navigator works to empower patients, improve access to sickle cell and other healthcare services to improve sickle cell disease outcomes. The purpose of transition is to prepare pediatric Sickle Cell patients for adult health sickle cell services. The navigator will be responsible for the implementation of the transition program including: identification of transition age patients, assessment of knowledge, education for patients and families and facilitation of transition. The Sickle Cell Navigator is an integral part of the interdisciplinary sickle cell team. The Navigator is also responsible to the establishment and implementation of the mentor program for young adults. The Navigator plays an important role in supporting health outcomes among sickle cell disease patients.

Required:
Must have good communication skills, basic computer knowledge
Experience working in a healthcare setting with the community

Shift- 8:30am- 4:30pm

Essential Functions:
Coordinate transition program to navigate patients from pediatric to adult sickle cell care
Perform transition readiness assessment & conduct non clinical education with sickle cell patients and families
Collect data regarding project activities and patients and input the data into project specific database
Document services delivered to patients in the EMR
Coordinate with Community-based organizations and Community Representatives to host Symposiums, informational sessions and trainings
Identify and troubleshoots patient compliance barriers
Navigate patients to primary physicians, specialty centers, community resources and entitlement programs
Coordinate support group programs for adult patients- including monthly support group, monthly yoga program, music therapy sessions and yearly sickle cell-ebration.
Coordinate education/support group programs for transition age patients.
Establish and manage mentor Program for transition age patients. Including recruitment, training, conducting continuous check ins, and conduct monthly peer mentor support group
Coordinate transition QI projects including: Transitions of Care initiative to leverage the SCD Learning Community and Data Hub to improve transition from pediatric to adult care by developing measures and evidence-informed quality improvement tools, which will be implemented and assessed at sites in the SCD Learning Community
