Job Description:

Develops the Plan of Care in conjunction with the patient, physician and interdisciplinary team to assume appropriate utilization of services and optimal client outcomes. Serves as case manager and assumes responsibility to coordinate patient care to maintain the highest level of care for comfort and quality of life as a goal. Attends IDT meetings to review hospice patient care plans. Participates in on-call rotation and provided appropriate patient's support. Applies nursing skills and techniques in the treatment of patients, providing emotional support and education to patients and families.

Desired Qualifications:

  • License as a Registered Nurse in PA
  • One Year Hospice/Homecare Experience Preferred, but not required
  • Valid Driver's License
  • Excellent Communication and Critical Thinking Skills
  • Understanding of Hospice Philosophy, Principles of End-of-Life Care
  • Self-Directed with the Ability to Work within a Team Setting
  • Criminal Background Check Clearance

Responsibilities: We are looking for compassionate, detail-oriented RNs to plan and organize hospice care for our patients in their homes and facilities.
