  • Assist Intake Coordinators on the equipment and supplies side as needed.

  • Receive and respond to incoming calls from referral sources and/or potential patients, home health agencies, account managers and exchanges information to identify the organization’s ability to meet the needs of the patient, on a case-by-case basis.

  • Contacts referral sources to advise of referral status.

  • Provides information regarding who will deliver the services to the clinical team.

  • Assist referral sources and/or patients with alternative community resources or equipment providers when the Company is unable to meet the needs of the referral source and/or patient.

  • Documents and route all calls in a manner consistent with the organization’s policies and procedures.

  • Collaborates with the Intake team management, account management staff, referral source and patients to identify potential solutions as problems are identified with payer sources.

  • Communicate customer service issues to management, as appropriate.

  • Identify, establish, and maintain correct patient and service account information.

  • Resolve patient complaints and coordinate correction of service to ensure service completion.

  • Deescalate patient calls and refers patients to appropriate department or person as needed.

  • Schedule deliveries, pick-ups, and accurately record file maintenances or changes in service.

  • Effectively communicate with patients of various ages and populations.

  • Accurately record every patient engagement in corresponding patient information system.

  • Maintain competency on incontinence coaching biannually.

  • Must meet individual and departmental performance metrics as indicated by manager.

  • Participate in traditional and e-learning programs.

  • Ensure compliance with Company policies and procedures.

  • Works effectively with other company employees, managers, and departments.

  • Performs all job functions with Company Mission, Vision, and Values in mind.

  • All other duties as assigned.
