Job Summary The Licensed Practical Nurse (LPN) provides culturally competent, trauma-informed, and person-centered care within our OASAS-licensed Outpatient Programs. This position requires comfort with both harm reduction and abstinence-based recovery models, recognizing that each client defines recovery on their own terms. The LPN supports clients in achieving their personal wellness and recovery goals. As part of a multidisciplinary team, the LPN will conduct medical assessments, administer medications, coordinate care, and facilitate service linkages with treatment providers, community resources, and health-related support services.

Key Responsibilities Conduct medical screenings both in person and via telehealth, review findings with a medical supervisor, and develop initial care plans. Provide client education, risk reduction counseling, and referrals for HIV, tuberculosis, viral hepatitis, sexually transmitted infections, and other communicable diseases. Serve as the Health Coordinator in alignment with OASAS guidelines. Support clients engaged in pharmacotherapy, ensuring proper medication management. Oversee the ordering, tracking, and administration of injectable medications. Maintain accurate and up-to-date documentation in compliance with OASAS clinic standards and electronic medical record (EMR) requirements. Address barriers to sustained recovery by fostering community partnerships and making referrals to health and social service organizations. Adhere to client confidentiality and ethical standards in accordance with Delphi Rises compliance plan. Maintain active professional licensure and certifications, ensuring timely renewals and completion of continuing education or training. Keep credentials compliant with organizational and regulatory requirements. Provide support to medical providers, including: Scheduling appointments and tracking missed visits Obtaining and reviewing blood lab results Assisting with prior authorizations Monitor program dashboards and conduct outreach as needed. Perform other duties as assigned.

Minimum Requirements Valid NYS LPN License Two years of clinical experience in a healthcare setting Skills & Competencies Proficiency in Microsoft Office (Word, Outlook, Excel) and experience with Electronic Health Records (EHR). Strong verbal and written communication skills to engage effectively with clients, colleagues, and external partners. Organizational skills, including the ability to manage multiple projects simultaneously. Analytical ability to gather and summarize data, identify solutions, and prioritize work effectively. Ability to develop and maintain accurate record-keeping systems. Knowledge of HIPAA regulations and commitment to maintaining strict confidentiality.
