The Lead Admissions RN has the primary responsibility of assigning bed placement for patients requiring admission to acute care and critical care units at the First Hill campus. This will include admissions from emergency departments, direct admits and inter-unit transfers. Surgical patients (in the case of add on procedures and/or changes to admission) as well as pediatrics, patients to CDU and admissions from outside facilities, will filter through this process. Chart review and/or calls to clarify bed criteria will be performed to assure appropriate bed placement. The Lead Admissions RN collaborates with nursing, physicians, EVS, the nursing resource office and other facilities to define, reduce and eliminate barriers within the system which may impede patient flow. Uses effective communication and interpersonal skills to collaborate with key persons and departments to facilitate and maximize throughput. Responsible for facilitating continuous throughput of patients from all points of entry. Ability to act independently and apply knowledge of hospital, unit admission criteria, and type of care provided in each department to appropriately place patients by utilizing critical thinking and problem solving skills. Evaluates and negotiates appropriate placement, prioritizing according to current census, staffing situations, and knowledge of patient acuity. Keeps detailed data on deferred and delayed patient admissions, providing weekly updates on patient placement issues. Determine the appropriate level of care and maintain communication with referral sources, families, patients and unit staff. Attends charge nurse huddle to understand inter-unit patient placement needs and staffing, and to communicate any patient placement needs or issues. Establishes good rapport and cooperative working relationships with personnel in the organization and the community for which Swedish serves. Maintain positive, collaborative interactions with physicians, department directors, and staff members to determine the needs of the facility and community. Communicates on an ongoing basis with environmental services supervisors to ensure bed turnover done in the right order for decreasing the wait time for a clean room. Communicates and coordinates with the Patient Logistics Coordinator to assign beds. Review system and census data on a daily basis. Assures accuracy in assigning and tracking the correct patient, physician/service, and type of bed on each admission. Effectively collaborates with unit leadership to problem solve issues related to admissions, transfers, and discharges. Respond to requests for information from nursing units, physicians, ancillary departments, as well as outside entities in order to assist them in completing their processes. Communicate pertinent information to ensure coordination of services and patient placement.
