Job Title: Remote Nurse Care Manager

Job Description

Join our team as a Remote Nurse Care Manager and play a crucial role in assessing, planning, and coordinating all aspects of medical and supporting services across the continuum of care for selected members. This role focuses on promoting quality, cost-effective care through daily management of member cases, facilitating communication between various care providers, and ensuring members receive comprehensive support for their socio-economic and medical needs. Successful candidates will manage a caseload of 75+ members, performing assessments, developing care plans, and actively participating in team care management rounds. This role requires a significant amount of outbound calls and coordination with Medicaid case managers.

Hard Skills

  • RN license

  • Experience in Case Management, Care Coordination, Utilization Management, Disease Management

  • Proficiency in managing Medicare and Medicaid cases

  • Ability to enter and maintain clinical information in medical management systems

Soft Skills

  • Excellent communication skills

  • Strong organizational skills

  • Ability to work independently in a remote setting

  • Proactive in reaching out to members and providers

  • Risk management and quality assurance awareness

Job Type

This is a contract position with a duration of 10 Weeks.

Work Site

This is a fully remote position.

Work Environment

This is a full-time, remote position, requiring availability Monday through Friday, from 8:00 AM to 5:00 PM PST. The schedule is not flexible, and the position is best suited for candidates living in the PST time zone. The role has a set contract period with a potential for extension, offering a structured yet supportive environment to facilitate professional growth and meaningful impact in healthcare management.
