SOUTH VALLEY SERVICES - PATHWAYS Community Housing and Case Manager in TOOELE, Utah

Title: Community Housing and Case Manager

Category:  Full time

FLSA Status: Non-Exempt

Pay:  $19.00 - $ 22.00 per hour (depending on experience)  

Schedule: Generally, Mon -- Thurs 800am -- 400pm (Advocacy Center); and Fri 800am -- 400pm (Shelter).

 

GENERAL PURPOSE

The Community Housing and Case Manager is responsible for working with Shelter staff and the Case Management team to identify individuals who are seeking shelter and then work with them to find long-term housing solutions. This position advocates on behalf of Shelter residents and assists community clients with case management, referrals, and support groups.   

This position provides secondary support for the Case Management and Resident Advocate teams.

South Valley Services is a non-profit organization whose mission is to provide options to anyone who has experienced domestic and sexual violence. SVS provides free and confidential services including emergency shelter, safe housing, trauma-informed counseling, advocacy, and tools for life.

 

ESSENTIAL DUTIES & RESPONSIBILTIES

·         Assist Shelter and community clients with housing needs and requests.

·         Identify all housing options (not just the Shelter) and community resources for clients.

·         Provide housing referrals to clients.

·         Enter and manage data for coordinated entry.

·         Attend weekly coordinated entry meetings (currently on Tuesday).

·         Participate in weekly case managers meeting.

·         Complete and send all TANF-related information and forms to the State of Utah and other agencies in a timely manner.

·         Enter TANF rent check requests in Vela.

·         Update all data/spreadsheets.

·         Enter clients in UHMIS within 24 hours of intake.

·         Assist with protective orders and enter them in State of Utah portal. Create/submit to courts and attend court with clients.

·         Co-facilitate community support groups virtually and in person.

·         Complete Case Manager duties at the Pathways Shelter.

·         Prepare, record, and file hotline information when received.

·         Under the direction of the Director, approve new Shelter residents per established guidelines.

·         Record, copy, file, and distribute new client information accurately/properly.

·         Engage in safety planning and follow-up meetings with all clients.

·         Demonstrate regular and reliable attendance. 

·         Ensure confidentiality and proper handling of all client information.

·         Maintain appropriate professional boundaries with residents, staff, volunteers, donors, and vendors.

·         Perform CPR and administer First Aid as needed.

 

OTHER DUTIES & RESPONSIBILITIES

·         Travel to Wendover, UT monthly to expand SVS services.

·         Attend Reentry Fair monthly in person.

·     & nbsp;   Attend all weekly/monthly staff meetings and trainings.

·         Attend events for the benefit of SVS as assigned.

·         Complete other duties as assigned.

 

QUALIFICATIONS & SKILLS       

·         Experience working with people (preferably in a human services field) and delivering excellent customer service.

·         Understand the definition, dynamics, and cause of domestic violence.

·         Able to stay organized while handling multiple tasks and competing priorities.

·         Must demonstrate strong problem-solving skills.

·         Must have a professional demeanor and demonstrate the ability to work effectively with a variety of personalities.

·         Knowledge of Microsoft Word, Excel, Power Point, and Outlook Programs.

·         Achieve and maintain eligible status on Direct Access Clearance System (DACS) Background Check.

 

PREFERRED QUALIFICATIONS

·         State of Utah Case Manager Certification

·         Bilingual English/Spanish

WORKING CONDITIONS & PHYSICAL DEMANDS

The environment is a residential shelter and office environment. At times, this position works at a desk using a computer and answering phone calls. This position, along with other Advocacy Center employees, is responsible for general upkeep of the Advocacy Center. This includes but is not limited to general cleaning duties and organizing/ distributing donations of food and clothing to community clients. The position reports to work at the Advocacy Center and the Pathways Shelter.

EQUIPMENT USED

Operation/use of general office equipment such as telephone, computer, fax, and copy machine. Use of general cleaning supplies and equipment.
