Pipeline Health System, LLC Discharge Coordinator Per Diem Day Shift in United States

Job Summary:

The Discharge Coordinator performs a wide variety of tasks and functions related to discharge planning, care coordination and resource utilization in order to assure the achievement of efficient and timely patient discharge. The Discharge Coordinator reports and works closely with the Case Managers and Social Workers.

Essential Job Duties:

  • Provides consultation to other staff, MD's, patients, significant others in the development of the discharge health care plan and the available community resources as needed

  • Evaluates the financial needs and insurance coverage of patients and assists patients and significant others in obtaining I applying for appropriate aid for discharge.

  • Collaborates with health care team when it is determined that the patients no longer benefits from acute treatment or when requested treatment is not available. Assists with coordination of transfers.

  • Serves as a resource for referral information (skilled facilities, home health agencies etc.

  • Serves as a role model for the Mission, Vision and Values of the organization and fulfills their job duties as requested within Scope of Practice

  • Maintains current competencies as well as updated knowledge in specific area of practice

Education/Experience:

  • Some college preferred

  • Minimum of 2 years in acute setting with at least 6 months as discharge planner/coordinator Case management

  • Clinical license a plus

Licensure/Certifications:

Current BLS for Health Care Provider card
