Standard Hours: Monday - Friday normal daytime business hours (n o on-call, no weekends and no holidays required). This is a field based position in the greater Washington DC area and you will be in the field 100% of the time going into members homes.

Primary Responsibilities:

  • Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care in their home

  • Manage members through an acute medical situation and work with chronic conditions, including testing, diagnostics, interventions

  • Identify, initiate, and follow up on referrals for community based programs and providers; including financial, psychosocial, community and state supportive services

  • Manage the care plan throughout the continuum of care

  • Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members

  • Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team

Required Qualifications:

  • Associates degree or higher

  • Current unrestricted Registered Nurse license in Washington DC

  • Certified in Basic Life Support

  • 1+ years of experience in post - acute care, such as long-term care

  • 1+ years of experience with using an Electronic Medical Record

  • Valid Driver's License

  • Access to reliable transportation

  • Ability to travel up to 100% to visit members in their homes within Washington DC

Preferred Qualifications:

  • 1+ years of experience working with the geriatric population

  • Experience conducting inservices

  • Field based experience going into patients homes

  • Experience creating care plans

  • Case Management experience
