Behavioral Health Registered Nurse Case Manager (RN) | Full-Time | Monday-Friday |Short Behavioral Health Visits | Coverage: Worcester Surrounding Area

As an RN with Elara Caring, you'll rock our world in the following ways:

  • Assess and Identify the needs of behavioral health care patients and their families, because understanding their quirks is part of the job.

  • Take Charge of your caseload like a boss, planning, coordinating, implementing, and evaluating treatment plans with precision and a dash of humor.

  • Craft personalized care plans that consider physical, psychological, social, and even spiritual factors. We believe in a holistic approach and some well-timed jokes.

  • Conduct initial evaluations and follow-up visits, ensuring that patients receive the care they need and a few good laughs along the way.

  • Communicate important findings, issues, or health condition changes promptly to the right people. Laughter may be the best medicine, but we take communication seriously.

  • Revise care plans based on patient care issues and collaborate with your amazing team to make sure everyone is in on the joke.

  • Provide counseling, instruction, and unwavering support to patients, facilities, and families. Laughter can heal the soul, and so can compassionate care.

  • Educate staff in alternate care settings about behavioral health philosophy and effective pain and symptom management. Time to share some laughs and knowledge!

  • Supervise and diligently document the delivery of care by behavioral health aides and LPNs. Paperwork may not be funny, but we make it as painless as possible.

  • Proactively report any unusual or potentially problematic nursing facility issues. We tackle challenges head-on, armed with quick wit and a can-do attitude. Offer compassionate support during times of bereavement, because sometimes a listening ear and a good chuckle are the best comfort.

Requirements :

  • Hold a current, unrestricted state RN license.

  • Bring a minimum of 1 year of experience in a clinical care setting as a nurse.

  • Demonstrate willingness to travel within the branch/office coverage area (up to 50%).

  • Possess a reliable vehicle, valid driver's license, and current auto insurance.

  • Preferred: Hold an Associates or Bachelors Degree in Nursing.

  • Preferred: Have previous Home Health or Hospice Experience.

  • Preferred: Possess previous experience with Homecare HomeBase. You know who you are, you Tech-savvy professionals!
