Position Details:

  • Schedule: Saturdays and Sundays (hours are flexible)

Key Responsibilities:

  • Assess and manage a caseload of behavioral health patients and their families.

  • Develop and implement personalized care plans.

  • Conduct initial and ongoing evaluations of patient needs.

  • Communicate significant findings and changes to the clinical team and other relevant parties.

  • Educate patients, families, and staff on behavioral health and symptom management.

  • Supervise and support behavioral health LPNs.

What is Required?

  • Current, unrestricted state RN licensee

  • 1+ years of experience in a clinical care setting as a nurse

  • Able and willing to travel within branch/office coverage area.

  • Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.

  • Must be able and willing to travel 50%

  • Associates Degree or Bachelors Degree in Nursing is preferred

  • Previous Home Health or Hospice Experience is preferred

  • Previous experience with Homecare HomeBase is preferred
