POSITION TITLE: Systems Navigator

DEPARTMENT: Outpatient

GENERAL HOURS

OF WORK: Monday through Friday, 8 a.m. to 5 p.m., some evenings and weekends

EXEMPT/NON EXEMPT: Exempt

SUPERVISOR: BHH Integration Specialist

POSITION SUMMARY: Responsible for the delivery of services as part of the Behavioral Health Home (BHH) initiative including oversight and maintenance of consumer core service delivery.

ESSENTIAL FUNCTIONS:

  • Performs all duties in accordance with the policies and procedures of the Center.

  • Ability to provide all six behavioral health home core services as outlined by MN Department of Health.

  • Responsible for updating and utilizing a patient registry in support of population health management.

  • Monitors and reports on client’s health status, quality metrics and target outcomes for the population.

  • Provide coaching to consumers and their identified supports.

  • Ensuring consumers are aware of and supported in efforts to access resources to address identified goals and needs.

  • Responsible for developing and nurturing relationships with community and social support providers to aid in effective referrals and timely access to services.

  • Assists and manages the coordination of work flow in quality improvement efforts to ensure positive consumer outcomes.

  • Ability to work effectively in a team environment, coordinating workflow with other team members and ensuring a productive and efficient work environment.

  • Monitor and track consumer contacts to ensure adequate billing requirements are met.

  • Participates in and is responsive to Center quality assurance procedures, cooperates with Center peer review and utilization procedures, and follows established procedures pertaining to maintenance of accurate and complete clinical records.

  • Available for evening and weekend work as required by Center needs.

    OTHER RESPONSIBILITIES:

  • Is sensitive to and supports management needs such as data collection and program evaluation, development of funding applications, maintenance of various contracts for services, etc.

  • Ability to travel extensively or as needed for consumer care.

  • Perform other work related duties as assigned.

    EDUCATION, EXPERIENCE AND SKILLS REQUIRED:

    Required Qualification

    When behavioral health home services are offered in a mental health setting, the systems navigator must meet one of the following qualifications:

  • A mental health practitioner as defined in Minnesota Statutes, section 245.4871, subdivision 26 or Minnesota Statutes, section 245.462, subdivision 17

  • A community health worker as defined in Minnesota Statutes, section 256B.0625, subdivision 49

    REQUIRED COMMUNICATION/MATHEMATICAL/REASONING SKILLS:

  • Must possess the required level of skills and abilities to read, write, speak, and apply mathematical concepts and sound reasoning to successfully perform the essential duties and responsibilities of this job.

  • Must be able to perform basic operations on a computer within the Windows operating system.

  • This position requires problem solving skills, critical thinking skills, and the ability to multi-task.

    REQUIRED PHYSICAL ABILITIES:

  • While performing the duties of this job, the employee is required to sit; use hands to finger, handle, or feel; and talk or hear.

  • The employee frequently is required to walk.

  • The employee is occasionally required to stand and reach with hands and arms.

  • The employee must regularly lift and/or move up to 20 pounds and occasionally lift and/or move up to 20 pounds.

  • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
